Dane County Department of Human Services, Children's Long Term Support waiver program
Wisconsin's Children's Long-Term Support (CLTS) Waiver Program (including the Katie Beckett Program and the Children's Community Options Program) makes Medicaid funding available to support children with substantial limitations due to developmental, physical, or severe emotional disabilities who are living at home or in the community. Funding can be used to support a range of services based on an assessment of the needs of the child and his or her family. The CLTS waiver is available to children or youth who meet several criteria. Founded in 2014, Harambee Village is a community-based doula agency committed to mothers, babies, and families. Access to prenatal support systems enhances relationships and strengthens our village locally and globally. At Harambee Village, we provide a safe space for birthing people to have access to the support, compassion, and health care that they deserve. Harambee Village is a community- based agency that consists of a team of trusted, compassionate, and diverse doulas, trained by the industry's leading organizations. Harambee Village has worked tirelessly to reimagine a new way of dismantling racial health disparities in maternal and child health. They have pulled their community together to begin improving birth outcomes in Wisconsin, through radical birth and reproductive justice work.

The Office for the Deaf and Hard of Hearing
The Office for the Deaf and Hard of Hearing (ODHH) is part of the Wisconsin Department of Health Services (DHS). The Behavioral Health Initiative (BHI) is focused on improving access to culturally and linguistically appropriate behavioral health services for people who are Deaf, hard of hearing, and Deaf-Blind.

Reach Dane provides high-quality early childhood services to over 1,000 underserved children ages 0-5 in Dane and Green County through center based and home visitation programs.
Roots 4 Change
Roots4Change (Raíces para el Cambio) is the first immigrant marketing cooperative in Dane County focusing on health. We are working to change the maternal and child health landscape to address inequities in perinatal well-being, by empowering families and challenging the system. Roots4Change (R4C) is owned by six independent social enterprises led by Latina and indigenous doulas and community health workers, who walk with families in their journeys of parenting, childbirth, motherhood, womanhood. Our ultimate goal is to develop a community leadership from within.
Wisconsin Family Connections Center
The Wisconsin Family Connections Center (WiFCC) provides equitable services for parents and caregivers partnering to raise children and youth through kinship care, foster care, and post-permanency, including reunification, adoption, and guardianship. Services include family events, support groups (both in-person and online), information and referrals, trainings and conferences, and case management. Triquestrian, LLC is a unique state-certified Outpatient Integrated Behavioral Health Treatment Clinic that specializes in creating safe, connected relationships to experience hope and healing. The Behavioral Health Resource Center (BHRC) is a voluntary person and family centered service designed to help all Dane County residents access behavioral health services in Dane County, regardless of insurance status, financial status, age, identity, ability or legal status. The BHRC assists Dane County residents from all ages, backgrounds and walks of life. BHRC services are consumer led and consumer driven, meaning that the individual and family is the expert regarding their needs.

BeSMART
BeSMART is a grassroots group of volunteers who seek to reduce gun deaths by teaching the tenets of our program: S (secure your weapons) M (model safe behavior around guns) A (ask others in your child's life if there are guns in their homes) R (learn to recognize possible suicidal tendencies in youth and young adults) T (tell someone in a position to help). We function under the auspices of Moms Demand Action and present and/ or table at community events, offering brochures, lively discussion, kids' activities, and often gun locks to give away.

Lions Clubs International is the world's largest service club organization with more than 1.4 million members in approximately 46,000 clubs in more than 200 countries and geographical areas around the world. Our Club was first chartered on September 18, 1957 with twenty three original charter members. Our members are from all walks of life joining in one common cause; to serve the needs of the local community. We celebrate the anniversary of that historic date in October of each year with a special dinner and program. We also have other fun social events throughout the year.
Mental Health Matters Coalition of Green County
Turning action into awareness, the Green County Mental Health Matters Coalition aims to spread knowledge of mental health resources available in the community and to reduce the stigma of substance use and mental health disorders. The Mental Health Matters Coalition falls under the umbrella of the Green County Healthy Community Coalition whose goals relate to the social determinants of health and needs assessments conducted in the community. The Mental Health Matters Coalition tackles relevant mental health needs in their community and surrounding areas with the support of community members and organizations that make up the coalition and would love to partner with you or your organization!

New Beginnings Chiropractic
Our prenatal, pediatric, and family neuro-focused chiropractic clinic has been serving the Mt Horeb community for over 16 years. Dr. Brenda Trudell, DC, has a degree in Psychology, and advanced prenatal and pediatric certifications with a focus on the nervous system. Understanding that the brain and nervous system control and influence all systems of the body, including mental and emotional health, is the foundation to our practice. Calming and balancing the nervous system, minimizing time spent in sympathetic/"fight or flight", and boosting the parasympathetic/relaxation and development arm of the nervous system, is what allows kids and adults to thrive, heal, and overcome challenges in today's world. The Verona Optimist Club is a voluntary organization of civic-minded men and women banded together for progressive thought and action in community service with special emphasis on service to youth in our community. Membership is composed of citizens drawn from business, industry, agriculture and the professions. Members are motivated by the desire to contribute to the community from which they and their families draw daily benefits. There is no greater opportunity for participation in community affairs and service than in the Verona Optimist Club. The club is a member of the Verona Chamber of Commerce and the Verona Community Betterment Association.
The BRAVE Research Center
The BRC is a neuroscience research lab at the UW-Madison Department of Psychiatry. We offer opportunities for caregivers and 10-16 year olds to participate in paid mental health research. We are exploring how trauma can affect brain development in youth, as well as social and biological functioning in families, to lead youth to resilience or vulnerability. With new information learned, we will create new treatments to help families in the future!
Three Gaits Inc. Therapeutic Horsemanship Center
Three Gaits offers hippotherapy which helps people with a primary physical or cognitive disability gain strength, coordination, confidence and independence through horseback riding and horse handling lessons. Located in Stoughton, Wisconsin, Three Gaits, Inc., is a non-profit, tax-exempt organization. Three Gaits strives to serve all interested people living in southern Wisconsin.

Treatment Assistance Fund, Inc.
We are a suicide prevention 501(c)(3) public charity based in Mount Horeb (actually, Daleyville!). We provide financial assistance to low-income people who need ketamine infusions for suicidal ideation but cannot afford them. We also work on raising awareness of ketamine within the public and also within the mental health profession.
Wellness By Intention
At Wellness By Intention, I offer holistic services for people and pets. For people, I provide Reiki and Craniosacral Therapy sessions, Raindrop Therapy, Red Light and Cold Laser Therapy. I also offer Canine Massage, Reiki, Red Light and Cold Laser therapies for dogs, as well as Reiki for other pets. I am an Animal Communicator providing a bridge between pets and their humans to gain a better understanding of behaviors, wants and needs. Reiki is given to the pet during a communication session which often leads to healing trauma and improving behavior. My office is located in the Innovation Center in Mount Horeb.
